Key responsibilities

Assess the social, emotional, and practical needs of individuals and families within the community
Develop and implement tailored support plans to address the specific challenges faced by clients
Facilitate access to community resources, services, and programs that can provide assistance and improve outcomes
Advocate on behalf of clients to ensure their rights and needs are met
Collaborate with other social service providers and community organisations to deliver a coordinated approach
Maintain detailed records and documentation to track client progress and outcomes
Participate in regular team meetings and training to continuously develop your skills and knowledge
