Key Responsibilities:
Manage and oversee care planning, delivery, and case management processes for seniors.
Develop and review Individual Care Plans (ICPs) to meet evolving needs.
Monitor care standards, ensure SOP compliance, and address escalated issues.
Engage with caregivers, next-of-kin, and external stakeholders to enhance community integration.
Manage coordination with stakeholders, operational data, and continuous improvement initiatives.
Oversee staff performance, compliance, training, and engagement to ensure quality care delivery.
Qualifications:
Diploma or degree in relevant fields (e.g., social work, healthcare).
Experience in client or patient care in community or eldercare settings.
This role focuses on managing and enhancing care services within a community care setting, ensuring quality and efficiency while supporting both clients and staff.
