This part-time role will involve providing essential social services and support to individuals and families within the local community. As a key member of our social services team, you will play a vital role in enhancing the wellbeing and empowerment of the communities we serve.
Key responsibilities
- Assess the social, emotional, and practical needs of individuals and families within the community
- Develop and implement tailored support plans to address the specific challenges faced by clients
- Facilitate access to community resources, services, and programs that can provide assistance and improve outcomes
- Advocate on behalf of clients to ensure their rights and needs are met
- Collaborate with other social service providers and community organisations to deliver a coordinated approach
- Maintain detailed records and documentation to track client progress and outcomes
- Participate in regular team meetings and training to continuously develop your skills and knowledge
