Job Summary:

  • Supports the Medical Social Workers and Expressive Therapists in delivering comprehensive services to the patients and their families.
  • Assists with administrative tasks, conduct assessments to facilitate patients’ interactions and contribute to the development and implementation of the intervention plans.
  • Renders psycho-emotional, social & spiritual care and support to Dover Park Hospice clients and in doing so, help enhance their quality of life and thus leading more meaningful lives.
  • Plays an active role in the planning and execution of the activities of the Hospice that promotes the objectives and the mission of the Hospice.
  • Assist clinical team with referrals of patients for both care and financial support to other community agencies (e.g. Home-Based Respite Care, Home Personal Care).
  • Collaborates with internal and external service providers to ensure that patients receive appropriate and timely support and follow-up services.
  • Survey community resources and collate the list of suitable care and financial resources to benefit patients and families.
  • Support coordination of community resources and services (e.g. HRHS List).
  • Maintains accurate and timely case notes and documentations.
Profile

Requirements:

  • For Social Work Coordinator, candidate should possess a Degree/Diploma in related fields (e.g. Counselling, Psychology, Sociology).
  • For Social Work Associate, candidate should possess a Diploma in Social Sciences (Social Work) or Social Services with a recognised institution.
  • Preferably 1 – 2 years of relevant experience in a healthcare environment and a total of 0 – 5 years of clinical experience.
  • Competent in MS Office applications especially in MS Excel.
  • Mature, dynamic and self-motivated.
  • Possess good communication and interpersonal skills as well as problem-solving skills.
  • Empathic, committed and enjoys working as a team.
